  • Overview





    Kearsley Rehabilitation and Healthcare Center is seeking an experience Social Services Director for our Nursing home facility.  Position will be responsible for the planning, organizing, developing and directing the overall operation of our Center’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations. 


    • Bachelor’s degree required. Master’s degree strongly preferred.
    • LSW licensure required; LCSW or LICSW strongly preferred.
    • Minimum  five (5) years experience as a social worker in a healthcare related setting. Experience within a nursing home or rehabilitation center strongly preferred.
    • Must be a supportive team member, contribute to and be an example of team work and team concept.
    • Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
    • Strong sense of urgency and willingness to be flexible to ensure all work is completed.
    • Strong, but comforting, communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
    • Must possess leadership ability and willingness to work harmoniously with and supervise other personnel



    • Interview resident/families as necessary and in a private setting to obtain social history.
    • Develop preliminary and comprehensive assessments of the social service needs of each resident.
    • Develop a written plan of care for each resident that identifies social problems/needs of the resident and the goals to be accomplished for each problem/need identified.
    • Ensure that social services components of the MDS are completed and signed on a timely basis.
    • Involve the resident/family in planning social service programs when possible in a private setting.
    • Refer resident/families to appropriate social service agencies when the Center does not provide the services or needs of the resident in a private setting. Develop and maintain a community and social services referral file of agencies and organizations that provide assistance to residents.
    • Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
