  • Overview





    Westgate Hills Rehabilitation and Healthcare Center is seeking an experienced Registered Nurse Assessment Coordinator (RNAC) for our Nursing home facility. We are seeking qualified candidates who have experience as an MDS Coordinator / RN Assessment Coordinator and are committed to help our resident and our facility receive the support they need.

    • Clinical Nursing Home: 2 years
    • MDS Coordinator: 1 year
    • RN with current active MD license
    • RAC-CT Certification preferred




    • Responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures.
    • Acts as in-house case manager by considering all aspects of the resident's care and coordinating services with physicians, families, third party payers and facility staff.
    • Accurate and thorough completion of the Minimum Data Set, Care Area Assessments and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.
    • Acts as an in-house Case ManagerProactively communicates with Administrator to identify regulatory risk, clinical trends that impacts resident care, and any additional information that has an effect on the clinical and operational outcomes of the center.
    • Collaborates with the facility’s staff to select the correct assessment of our residents.
    • Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care and Medicaid.
    • Ensures timely submission of all Minimum Data Sets to the state data base and ensures that the necessary follow-up action is taken.
    • Facilitates the Care Management Process by engaging the resident and all other appropriates staff members.
    • Analyzes QI/QM data with the Director of Nursing to identify trends on a monthly basis.
    • Serves on, participates in, and attends various committees of the facility.
    • Responsible for the CMI.
