Overview

Marquis Health Services is seeking a qualified Case Manager/MDS Coordinator. This position is responsible for ensuring the development and completion of the resident assessment (MDS) in accordance with the requirements of this state and the policies and goals of the facility. This person serves as a key member of the center’s management team by providing and the helping the center provide quality care for Medicare, Medicaid (Case Mix) and Managed Care patients.

 

Please note, this position will be considered a floating/traveling position. This individual will cover facilities throughout Massachusetts and Providence, Rhode Island.

Qualifications

  • Graduate of an accredited School of Nursing, (RN, BSN or LPN)
  • Current/active licensure for Registered Nurse.
  • Minimum 3 years clinical experience in a long-term care setting.
  • Prior MDS/RAI experience preferred
  • Demonstrated clinical assessment skills.
  • Knowledge of Medicare and Medicaid regulations and coverage guidelines as related to skilled nursing.
  • Required to travel throughout the MA/RI region.

Responsibilities

  • Conducts patient assessments to determine the patient’s prior level of function to establish patient centered goals, treatment plans that focus on recovery and discharge planning
  • Assesses and monitors the patient on a daily basis to promote patient recovery and provide quality care
  • Provides patient education on disease management, medication management, and factors that effect safe discharge
  • Facilitates discharge planning with the patient, family by coordinating the planning from admission through discharge with the physician, nursing, rehab, social services and nutrition services team members
  • Advocates on behalf of the patient and center for needed patient resources and services within the clinical needs of the patient and the reimbursement continuum
